There are no cards for your dental or optical coverage. The provider would have to contact C&R consulting at (212) 869-5986 or (212) 366-9339 for the breakdown of dental benefits using policy holder’s social security number. For the optical the provider would contact GVS or CPS.

Can I get reimbursed for my dental or optical claims?

Yes, you can get reimbursed. You would need to send a detail receipt which would include the Date of Service, Name of Patient and the Codes/Description of the procedures done. For Optical you can get up to $100 reimbursed, but for Dental it depends on the In-Network fee schedule amount. When am I eligible for medical benefits?

Plan A: If you work under a CBA (Contract) that requires your employer to pay a monthly premium to the Fund on your behalf, you will become eligible for coverage the first day of the month following the month in which contributions are reported. For example: If your employer is first required to contribute to the Fund on your behalf for the month of January, your employer will have to report the premium obligation and/or make the required contribution in February, and your coverage will commence on March 1.

Plan B: If you work under a CBA (Contract) that requires your Employer to make hourly contributions to the Fund, you will become eligible for coverage once you have worked 250 hours in Covered Employment (i.e., work for which your employer is required to contribute to the Fund on your behalf). These 250 hours will “buy” you a calendar quarter (three months) of coverage. If you work at least 250 hours during one of the following periods, you will be covered for the calendar quarter beginning on the date shown below:

April, May, June July 1

July, August, September October 1

When do my benefits get terminated?

Plan A: Coverage will be terminated the last day of the second month following the month for which the final premium was due to the Fund on your behalf. For example: If you are laid off on May 15th and your employer’s last premium was due to the Fund on your behalf for May, your coverage will be terminated on July 31st unless you elect and pay for COBRA coverage.

Plan B: If, during any given quarter, you work 200 or more hours, but less than 250 hours, you will have the opportunity to “buy” the remaining hours needed to become eligible for coverage. The cost to purchase hours needed for eligibility will be $6.00 per hour.

What do I need to provide for the initial process of my Pension Application?

The process consists of submitting all Application Forms (Covered Work Employment, Return to Work Rules, and Signed Application) along with all the necessary documents such as:
  1. U.S. State ID, License, or US Passport
  2. Social Security card(s)
  3. Marriage Certificate (if applicable)
  4. Birth certificates (original and translated (if in a different language))
  5. Divorce Decree (if applicable)
  6. Death Certificate (if applicable)

What happens after I submit my initial documentation for my Pension Application?

Once reviewed, we will determine if you are eligible for a Pension Benefit. If eligible, we will commence the calculation process, in which we review the work history and determine the monthly benefit. When the Monthly Benefit has been approved, you will receive a Declaration of Retirement or an Option Package to elect your benefit and finalize the application. Will I continue to have Medical Coverage once I start receiving my pension?

We do not offer Medical Coverage to our pensioners. If you were actively working upon retiring with us, please refer to the following:
  • For Plan A members, your coverage will be terminated the last day of the second month following the month for which the final premium was due to the Fund on your behalf.
  • For Plan B members, your coverage will be terminated once your hours fall below 250 hours.
  • COBRA could be elected if you have not reached age 65.

Members will always receive payment on the 1st of each month. If the first falls on a weekend or on a holiday, the member will receive payment the following business day.

Once you receive the initial payment, the Single Life option elected cannot be changed.
For a spouse to be considered a Surviving Spouse, you must have to be married at least 12 consecutive months prior to commencing Pension.

Once you have commenced your Pension Benefit, you will not be able to add your current spouse in place of your previous spouse, regardless if they are deceased or divorced.

How do I know what my effective date for my benefit is?

Your effective date is the 1st of the Month following the month in which your application was received, or the 1st of the Month following the month you turned of Age for Retirement. If you are over 70, we follow the RMD rule which is effective April 1st following the date you turned 70.5 or 72.
