You would need to mail, fax or email a Judgement of Divorce/Divorce Decree, Death Certificate, or lose coverage when your medical coverage is terminated.
How can I add a dependent to my insurance?
Fill out a dependent enrollment form and fax or mail the required document noted on the form. • Spouse: Marriage Certificate, Birth Certificate if the Marriage Certificate does not have his/her date of birth, and a copy of their Social Security Card • Children: Birth Certificate and a copy of their Social Security Card
